Apache Tribe of Oklahoma Burial Assistance
Providing help after losing a loved one, the Apache Tribe of Oklahoma Burial Assistance offers financial assistance for funeral services and meals. The Burial Assistance provides up to $6,000 for tribal members’ funeral costs. The payment is issued directly to the funeral home after all required documents (completed application, funeral home W-9, final invoice) have been submitted.

- ____Funeral Home Invoice/Statement of Funeral Goods and Services
- ____Funeral Home W-9
Responsible Party: Person(s) who signs the burial contract/funeral home statement with the funeral home is considered the responsible party and must fill out and sign the ATO burial application.
