2026-2027 School Clothing Assistance Application

The Apache Tribe of Oklahoma will provide school clothing assistance to Apache Tribal students who are enrolled in Kindergarten to 12^th grade. Each student will receive $200 gift card, which is to be used for school clothing, and $100 gift card to be used for school supplies.

Application deadline is August 31, 2026

Requirements for Assistance:

A Verification of School Enrollment Letter, signed by a school official, is required. The letter must include the parent/guardian’s name and the student’s name and grade for the 2026-2027 school year. The letter must include the print name and signature of the school official completing it and be on a school letterhead.

Rights and Responsibilities

Parent/legal guardian acknowledges that the funds distributed through the Apache Tribe of Oklahoma are solely for the purchasing school clothing for the applicant. Notice Regarding False Statements Whoever, in any matter within the jurisdiction of any department or agency of the United States, knowingly and willfully falsifies, or covers up by trick, scheme, or device a material fact or makes a false, fictitious, or fraudulent statement or representation or makes or uses any false writing or documents, knowing the same to contain any false, fictitious or fraudulent statement or entry, may be punished by fine, imprisonment, or both. (8 U.S.C., Section 1001) I have read and understand the preceding Rights and Responsibilities, Notice Regarding False Statements, and the program guidelines. I verify that I am a custodial parent/legal guardian for the minor listed above or have been given permission to receive and spend the funds on behalf of the above named tribally enrolled student by the custodial parent/legal guardian.
